E-Forms Instructions

 

Introduction

To enhance the operation and administration of the Telecommunciations Service Priority (TSP) Program, the TSP Program Office (PO) has developed a TSP electronic forms (e-forms) application that is accessible via the TSP website on the Internet. The e-forms application provides TSP Program users and vendors with a user-friendly mechanism for performing required TSP processes and enables the TPS PO to process TSP information more efficiently and effectively. TSP Program users and vendors can use the e-forms application in lieu of the paper forms to perform tasks securely, such as completing and submitting requests for TSP restoration priority level assignments, fulfilling information processing and reporting requirements, and maintaining records. The following processes are available through the e-forms application:

  • Users
    • TSP Request For Service (SF 315):(Restoration requests only)
    • TSP Revalidation (SF 314)
  • Vendors
    • TSP Service Confirmation (SF 318)
    • TSP Service Reconciliation (SF 319)

User System Requirements

The e-forms application resides on a secure server that is accessed via the TSP home page (http://tsp.ncs.gov). To use the e-forms application, you must run a version 4.0 or higher Web browser, such as Netscape 4.0 or Microsoft Internet Explorer 4.0, that has the JavaScript functionality enabled. In addition, an e-mail address is required to receive notices from the TSP PO about your e-forms activity. Note that before you can use the e-forms, you must register via the TSP Web site and receive a login and initial password from the TSP PO.

Registering To Use The E-Forms Application

You can register to use the e-forms application via the TSP home page, and specifically the e-forms main page. This process enables you to receive a login and password from the TSP PO. To register to use the e-forms application, perform the following steps:

  1. Access the TSP home page at http://tsp.ncs.gov and click on the e-forms link in the left margin of the screen. The server displays the e-forms main page.
  2. Click on the Online Registration button at the bottom of the screen. The server displays the E-forms Registration window.
  3. Complete each of the following fields:

    Name* Title Organization
    E-mail address Phone Number** Address
    City State Zip


    * Use the format Last name, First name.
    ** Use the format xxx-xxx-xxxx.
  4. Select the radio button that describes your organization type. Choose from the following:
    1. Federal Government
    2. State Government
    3. Local Government
    4. Private Sector
    5. Foreign Government
    6. Other User
    7. U.S. Military
  5. Provide a justification for registering to use the TSP e-forms application in the Justification field. If you are a current TSP Program user or vendor, state that information here

  6. To clear the form, click on the Reset button.
  7. To exit the registration form without submitting it, click on the Back button of your Web browser.
  8. To submit the registration, click on the Submit button. The system notifies you of any formatting errors in your registration information. After correcting the error(s), click on the Submit button again to send the information to the TSP PO.
  9. The system displays the Submit Registration window, as shown below:

  10. You should receive your login and password from the TSP PO via e-mail within 5 business days of submitting the request. If you do not receive this information, contact the TSP PO at 703-676-2255.
  11. Click on the Please Click to Continue… link to return to the E-forms main page. Use the navigation buttons on the top and left portions of the screen to navigate the TSP site.

Login Procedures

The TSP e-forms application provides easy point-and-click access to the e-forms. E-forms users can login directly to the application via the TSP home page. Use the following procedures to access the e-forms. To login to the TSP e-forms application, perform the following steps:

  1. Access the TSP home page at http://tsp.ncs.gov and click on the e-forms link in the left margin of the screen. The server displays the e-forms main page.
  2. Click on the E-forms button at the bottom of the screen. The server opens a new browser window and displays the E-forms Login window.
  3. Enter your login and password in the Name and Password fields. Note that both fields are case-sensitive.
  4. Click on the Login button to login to the e-forms application

  5. If your login or password is incorrect, the e-forms application instructs you to try again. Make sure that the Caps Lock key on your computer keyboard is not on and enter your login and password again. Note that after three unsuccessful attempts, the e-forms application locks the user account and instructs the user to contact the TSP PO to reset the password.
  6. If your login is successful, the e-forms application displays the E-forms Main window, as shown below. The TSP POions that are displayed are dependent on your login status as a user or vendor. If this is your first time logging in to the e-forms application, you are required to change the initial password provided to you by the TSP PO and login again using the new password.

    Note: If your e-forms session is inactive for more than 30 minutes, the e-forms application automatically times out. To continue, you must login again and reenter the information that was not submitted to the TSP PO during the previous session.

Login Procedures

TSP e-forms application users can logout of the e-forms application at any time via the e-forms menu bar shown below. The e-forms menu bar is located at the lower left portion of the screen on each page of the e-forms application. Note that all information that has been entered into a form is erased each time you logout of the e-forms application unless it has been submitted to the TSP PO.

To logout of the TSP e-forms application, perform the following steps:

  1. Click on the Logout link of the e-forms menu bar on the lower left portion of the screen. The e-forms application returns you to the E-forms Login window.
  2. Close the browser window by clicking on the X in the upper right corner of the window. The system displays the e-forms home page.

Changing E-Forms Passwords

You are required to change the initial password that you receive from the TSP PO the first time you login to the system and on a periodic basis thereafter. The system will prompt you to change your password every 6 months. Note that the password must be composed of between 8 and 11 characters and include three of the following items:

  • Uppercase characters
  • Lowercase characters
  • Numeric characters
  • American Standard Code for Information Interexchange (ASCII) characters (i.e., punctuation marks, special symbols)

To change your e-forms password, perform the following steps:

  1. From the E-forms Main window, click on the Password Change link on the e-forms menu bar. The e-forms application displays the Password Change Form, as shown below:

  2. Type your current password in the Old Password field.
  3. Type your new password in the New Password field, and type the password again in the Re-enter Password field to confirm your password entry.
  4. To clear your entries, click on the Reset button.
  5. To exit the Password Change Form without changing your password, click on the Home link on the menu bar in the lower left portion of the screen. The e-forms application returns you to the E-Forms Main window.
  6. Click on the Submit button to change your password. If there are errors in the Password Change Form, the e-forms application displays a blank password change form and instructs you to correct the errors.
  7. If there are no errors in the Password Change Form, the e-forms application informs you that your password has been changed. The new password will be in effect the next time you login to the e-forms application.

Accessing the E-Forms Online Help System

In addition to the instructions provided in this manual, you can access the E-Forms Online Help System for a description of form fields and field requirements. To access the E-Forms Online Help System, perform the following steps:

  1. Click on the Help link on the e-forms menu bar
  2. The e-forms application displays the E-Forms Online Help System in a new browser window.
  3. Click on the appropriate link to display the instructions for the field in question.
  4. Close the browser window by clicking on the X in the upper right corner of the window. The e-forms application is displayed.
